InRoads Program
Supporting the community’s most vulnerable to reconnect with health and social services with help from our Community Connectors.
The InRoads Program is a non-judgemental, trauma-informed, low-barrier model of care seeking to enhance the health outcomes for those at risk of, or experiencing, homelessness.
Our Community Connectors meet people where they are at, building trust and providing practical, relationship-based support when it’s often needed most.
They provide one-to-one support and help to bridge the gap of systemic barriers through:
- Connection – by working at an individual’s pace to build rapport and explore health needs.
- Health access – through supporting people to connect with healthcare.
- Attending appointments – by assisting with bookings, reminders, preparation, and attending appointments.
- Navigation – by helping people move through complex systems.
- Central coordination – by acting as a central point of contact across health, housing, and other support services.
- Engagement – through gentle re-entry into healthcare via our Nurse Practitioner and GP.
